Please mark below the lunches that you are purchasing.  Payment of $4.00 per lunch must be sent in with this order.  Parents can purchase lunches, but they must also be pre-ordered and prepaid.  Please note that this order form and money must be turned into your child’s teacher by this Friday, February 24. MAKE SURE YOU SEND IN A SEPARATE ORDER FORM FOR EACH OF YOUR CHILDREN.
